Workers Comp

 

UIA is the residual agency for WCF in Utah.  

We provide access to WCF for all agencies and will gladly assist you in securing coverage for your clients.  

Follow these steps for assistance:

1 - Email Matt Child a completed Workers Comp Acord 130 Application and Loss Runs for your client.

2- Matt will will review the application and submit it for consideration.

3 - Once a proposal is received, it will be reviewed by UIA, and forwarded back to you for presentation to your client.

4 - To bind, email a formal request to bind, with the desired effective date, along with a client signed copy of the proposal. 

5- UIA will confirm coverage is in place via return e-mail.

6 - Your cient can then make a payment at www.WCF.com

  • FEIN
  • All specific requested codes
  • Payrolls and employees under each code
